This list is not all-inclusive. Services are tailored to each individual following a thorough nursing assessment. Personalized care plans are developed to ensure every resident’s unique needs, preferences, and well-being are thoughtfully addressed.
Following a nursing assessment, each resident’s care plan may include, but is not limited to, support with dressing, grooming, toileting, showering, mobility and ambulation, exercise, and other activities tailored to their personal needs.
Staff can assist residents with setting up medical and non-medical appointments (hairdresser, nail, etc.).
Staff can assist residents in arranging medical and non-medical transportation as needed. When available, medical transportation may be covered under the resident’s insurance and will be coordinated by staff. If insurance does not cover medical transportation, the cost will be the resident’s responsibility. All non-medical transportation costs are the responsibility of the resident.
Call-light systems are available for residents to easily alert staff, ensuring timely and appropriate responses to both requests and emergencies.
Following a nursing assessment, residents may receive medication assistance through reminders, setup, management, or staff administration, as appropriate. This approach helps ensure medications are taken safely, accurately, and in accordance with provider orders.
We offer a monthly vital check for all residents. More frequent checks can be added based on the Resident's healthcare need.
